Note - Pre-K and K students may not require transcript or test scores

• Copy of birth certificate
• Copy of social security card
• Transcript from previous school(s)
• All academic records including standardized test scores, gifted services, IEP services, and/or special education records
• Current psychological evaluation (if applicable)
• Immunization record
• Certificate of Vision, Hearing, and Dental
• Attendance record for previous year
• Discipline Report (if applicable)
