Four Simple Steps for Participating in the Program
	#1: Complete Frederica Apogee Scholarship Fund Form. Please check the box giving Apogee officials authorization to coordinate your donation with the Georgia Department of Revenue. #2: Simply sign Section C, Page 2 of the Georgia Tax Credit Form (Form IT-QEE-TP1). With your signature, Apogee officials will complete and submit the form on your behalf. #3: Write a check made payable to “Apogee Scholarship Fund” and mail to: Apogee Georgia School Choice Scholarship Fund 3330 Cumberland Boulevard, Suite 400 Atlanta, Georgia 30339 #4: Retain the original documentation you receive from the State of Georgia so that you can give it to your tax preparer when he or she processes your state and federal tax returns.
