Frederica Academy enhances the wellness of each student and employee by attending to the physical, mental, emotional, and spiritual aspects of each person. The school nurse can be reached at extension 492.

Our school nutrition program offers a variety of lunch items each day. Vendors bring in meals to those students who have ordered ahead of time. Salads, yogurt, fruits and other items are also available. Our vending machines in Hamilton Hall and the Upper School have waters, juices, and sports drinks, as well as healthy snacks.

Health Forms Needed for School Attendance

Georgia Form 3231—Proof of Immunization—
 All students entering Pre-Kindergarten, Kindergarten, or 6th grade and all new students from another state entering any grade level should see their physician (or the Glynn County Health Department) to make sure they are up to date with immunizations. Students entering Frederica Academy from another state need to have their immunization records from that state reviewed by a GA physician or the Health Department and obtain the GA form.  From time to time we will audit our immunization records and will notify you if there are vaccines due.

Georgia Form 3300- Proof of Eye, Ear, and Dental Exam- 
This form is required for all students entering school in Pre-Kindergarten or Kindergarten, and for all students entering at any other grade level who are from another state.  

Student Health Information & Consent Form – Provides health and parent contact information for the entire school year, including field and sports trips. Please review and update carefully.  This form is mailed each summer, is mandatory for every student, and is due on the first day of school.  Please call the school nurse with all student health information changes and updates.

Athletic Screening Form— If your child plans to participate in our Middle and Upper School athletic programs, they must have a physical prior to the first practice.  Our Booster Club sponsors on-campus screenings every spring.  These screenings cover the student for participation in sports the following school year.  

Prescription Medication Form – is necessary if your child is on any long term prescription medication that will be given either during the school day or on field or sports trips. A physician’s signature is required. If this form applies to your child, it is due on the first day of school or on the first day of a new long term medicine. Blank forms are available on the website on in the front office.

If your child will be taking a short term prescription (such as a 10 day course of antibiotics), we must have a note from the parents directing that the medication be given at school. Specific information regarding medicine name, dosage and time to be given should be clearly stated in writing

All prescription medication must be provided in a medication container with the prescription on the label. All medication must be delivered to the school nurse, teacher or to the receptionist in the front office.
