Here is a list of forms (
one form per each student) you will need to complete and submit:
- The Consent for Treatment should be submitted to Nurse Byers. Click here for form.
- The Network and Internet Acceptable Use Agreement should be submitted to Mrs. Dottie Barrow in the Media Center. Click here for form.
- (Middle and Upper School) The Frederica Academy Honor Code should be submitted to Mrs. Lavallee in the Upper School. Click here for the form.
- (Lower School) The FAST form should be submitted to Monica Guza. Click here for the form. FAST is available starting the first day of school, August 15. If your child will be participating in the FAST program on the first day of school, the form must be submitted before August 15.
